Fall Nursing Program Entrance Requirements Form

Before you begin filling in the form below you will need electronic copies of your transcripts.  Applicants who have attended more than one college, are required to submit ALL college transcripts to determine a cumulative GPAA cumulative GPA will be determined by the Registrar.

You may use unofficial transcripts for this part of the application process.  Please note:  You must also have ALL official transcripts sent directly to FM.  This includes your high school transcript or a copy of your GED/TASC Scores.

This Fall 2022 Program Entrance Requirements Form must be submitted to move forward in the admission process. Attach transcripts to this form before clicking submit. Incomplete forms will not be reviewed.

If you have met all of the prerequisite requirements and you have submitted the form successfully, please be sure to complete the Fall Online Nursing Application.

  • High School prerequisite courses cannot be more than 5 years old. If they are more than 5 years old, you must complete the prerequisite courses at the college level.
  • Choose the grade you received in this course. A minimum grade of "C" or above is required (Statistics does not fulfill this requirement). For complete course details visit fmcc.edu.
  • Tell us the name of the course you completed, where you took the course and the date/semester that you completed it.
  • Choose the grade you received in this course. A minimum grade of "B" or above is required for this course (Anatomy & Physiology and Microbiology do not fulfill this requirement). For complete course details visit fmcc.edu.
  • If you are using High School Living Environment to fulfill this prerequisite you must have a regents exam grade of 80% or above.
  • Tell us the name of the course you completed, where you took the course and the date/semester that you completed it.
  • Choose the grade you received in this course. A minimum grade of "B-" or above is required. For complete course details visit fmcc.edu.
  • If you are using High School Chemistry to fulfill this prerequisite you must have a regents exam grade of 80% or above.
  • Tell us the name of the course you completed, where you took the course and the date/semester that you completed it.
  • You must have a cumulative minimum GPA of 85% and not more than 5 years old to qualify for this prerequisite.
  • You must have a cumulative minimum GPA of 2.8 to qualify for this prerequisite. Applicants who have attended more than one college, are required to submit all college transcripts. A cumulative GPA will be determined by the Registrar.
  • THIS IS NOT A REQUIRED PREREQUISITE. Choose the grade you received in this course. A grade of "C" or above is required. For complete course details visit fmcc.edu.

  • You must Upload unofficial copies of all your transcripts to this form in order to move forward in the Nursing application process.
