The following checklist will assist you in preparing your paperwork and registering for classes at Fulton-Montgomery Community College.
_____ FM Application
Fill out and return to the Admissions Office.
_____ One Official High School Transcript (signed/sealed)
showing date of graduation. If you are not a high school graduate, we will need a copy of your GED AND a copy of your official high school transcript. Have your High School remit the transcript directly to the college. If your name has changed since graduation, have your High School include your current name and Social Security # on the transcript.
_____ Proof of Immunization
is required if taking 6 or more credits. (See Immunization Requirements)
_____ Certificate of Residence
notarized at FM’s Bursar’s Office. If you are NOT a Fulton or Montgomery County resident, it must be notarized at your County Treasurer’s Office.
_____ College Placement Assessment Testing & Advisement Registration

_____ Free Application for Federal Student Aid (FAFSA)
Complete and return to the Financial Aid Office. Direct questions to the Financial Aid Office at (518) 736-FMCC x8201. Students not possessing a high school diploma or GED must successfully complete the Ability to Benefit (ATB) test prior to registration in order to be considered for Financial Aid. Please call the Student Development Center to schedule an appointment for ATB testing.
_____ Transfer Students need to request official academic and financial aid transcripts from any previously attended colleges. Request that both documents be sent directly to FM.
_____ Campus Tour or Catalog
Contact our Admissions Office at (518) 736-FMCC x8301.
_____ If you have earned AP or college credits, submit an official transcript to FM.
_____ Student Identification Cards-Available in the Public Safety Office, P-144, Physical Education Building.
All members of the campus community to include students and employees are required to have a valid FM identification card that can be produced upon request. The student identification cards can be obtained in the Public Safety Office, P-144 (Physical Education Building). After your schedule has been completed you can stop by or contact the Public Safety Office at 736-FMCC x4000 to inquire about scheduled dates and times for ID processing.
