Testing Accommodation Procedures

This information will assist students who are eligible for accommodations related to a disability.  Students requesting disability accommodations must provide documentation of their disability to the Accessibility Coordinator, Robin DeVito.  After meeting with Robin please come to The Alternative Testing Center and Academic Success office, which is located on the second floor of the Evans Library to set up your accommodations.

Loans for assistive technology are now approved through the Academic Success office and the devices must be checked out at the library desk. Ellie Fosmire, Academic Success Coordinator is located in L203A on the second floor of the library.

Testing Accommodation Procedures:

Make sure you have documentation on file supporting your testing needs.

The electronic version of the test request form is accessible through the FM web page.

Click on MyFM then click on Academic Services and log in with your username and password.

You can access your professor signature sheet by visiting the alternative testing site.

You must return your signature sheet before you can put in test requests.

Complete the form at least 2-3 class days before the test.  That means Monday tests need to be scheduled by Thursday the week before.

Click on new request.  Click on the class and section you are asking accommodations for ex. SOC100A.

Select the date the test will be given and the date you will be taking the test.  (These are often the same date unless you have night classes or makeups).  The test request will be delivered electronically to your instructor and the testing center.

When you arrive to take your test

Your instructor sends the test to the Learning Center with detailed instructions.  We will follow the test instructions in proctoring the exam.  Sometimes we need to check with instructors if you ask to use notes, books, calculators etc.  What they say goes.

Be on time

Take the test at the Learning Center during your scheduled test time.  If you arrive after the class has taken the test you will have to reschedule your exam with the instructor’s permission.  You cannot leave the testing center until your test is complete unless you have made arrangements in advance.  That means no bathroom breaks (go before you start the test), no phone calls etc.

The Learning Center will hold your test until your instructor arranges to pick it up.  Some instructors ask for the test to be delivered through the FM email.
